Submit Certified Copies of Qualifications

  • You’ll need to provide certified copies of your Qualification Certificates and Academic Transcripts. In the event that your Qualification Certificate has not yet been issued, we are pleased to proceed with your Academic Transcript in the interim.
  • These must be certified by a Justice of the Peace (JP). You can find a JP in your state or territory through the provided links.

If you are unable to access a JP, documents may be certified by any of the following professionals: (Architect, Chiropractor, Dentist, Accountant, Legal Practitioner, Medical Practitioner, Postal Office Worker, Police Officer, Nurse, Occupational Therapist, Optometrist, Pharmacist, Physiotherapist, Psychologist, Veterinary surgeon)

College Letter Requirement – Remedial Massage, Myotherapy, Chinese Massage

For the following modalities, a signed college letter is required to confirm completion of 200 supervised, on-campus, face-to-face clinic hours. Please have your college complete the attached health fund letter and return it with your application.

Health Funds currently requiring this letter:

  1. Remedial Massage

    • College Letter required for: Medibank, HCF, ARHG, AHM

  2. Myotherapy

    • College Letter required for: HCF, ARHG

  3. Chinese Massage

    • College Letter required for: Medibank, ARHG

Registration-Cert

Submit AHPRA Registration for These Modalities: Acupuncture, Chinese Herbal Medicine, Chiropractic, Osteopathy

Applicants for Acupuncture, Chinese Herbal Medicine, Chiropractic, and Osteopathy are required to provide a copy of their current AHPRA (Australian Health Practitioner Regulation Agency) registration. ATMS requests that you submit a copy of your renewed registration along with your application.

Submit Your Provider First Aid Certificate:

  • Accredited members who wish to have health funds provider status are required to have a current Senior First Aid certificate. Please ensure a copy of the certificate is included with your application form.
  • First Aid needs to be renewed every 3 years. The course is officially called Provide First Aid HLTAID011.

4. Health Fund Recognition (if applicable):

To be eligible for health fund provider status, an Accredited Member of ATMS must:​

  • Fill out the Health Fund Application & Declaration Form Click Here
  • Be in private practice.
  • ATMS and the Health Funds define Private Practice as ‘Independent private practice means a professional practice (whether sole, partnership or group) that is self-supporting. This means that its accommodation, facilities and services are not provided or subsidised by another party such as a public hospital or publicly funded facility.
  • Have current Provide First Aid on file with ATMS at all times.
  • Have current Professional Indemnity Insurance on file with ATMS at all times
  • Comply with the ATMS Continuing Professional Education (CPE) Policy
  • Have a clinic address (mobile services must have an invoice address and Full street address, no PO Box addresses are accepted)
  • Hold accreditation with ATMS in the relevant modality.
  • Meet the health fund provider status requirements for that modality, which may differ between funds.
