Malignant melanoma is noted for its particularly aggressive clinical behaviour, a propensity for lethal metastasis and striking therapeutic resistance. It occurs when melanocytes transform into a highly migratory population that give rise to cells of multiple fates. Recent years have brought an increased interested in nutritional and botanical dose-dependent intervention for the prevention of melanoma. Although randomised human controlled trials are lacking, basic science and epidemiological studies show promising benefits in the treatment and prevention of melanoma. Some critical aspects of current cancer chemotherapy are also presented.

Clinical pearls:

*  Biochemical and pathophysiological considerations
*  Epidemiology
*  Conventional treatment: monoclonal antibodies and immune-modulating antibodies.
*  Natural compounds that inhibit or down-regulate COX2, EGFR, TNF-alpha, PI3K
*  Natural compounds that normalise p53, E-Cadherins, MAPK
*  Dose-dependent evidence-based research for an effective management of melanoma.
